腹腔镜胃十二指肠溃疡穿孔修补的优势和手术效果(1)
【摘要】 目的：研究腹腔镜胃十二指肠溃疡穿孔修补的优势和手术效果。方法：收集2015年4月-2016年11月胃十二指肠溃疡穿孔患者46例分两组，开腹手术组进行传统手术治疗；腹腔镜手术组实施腹腔镜胃十二指肠溃疡穿孔修补。就两组患者胃肠功能正常的时间、从手术结束到离开医院时间、手术过程出血量、术后平均引流量和肠梗阻等手术并发症率进行比较。结果：腹腔镜手术组肠梗阻等手术并发症率明显低于开腹手术组(P<0.05)。腹腔镜手术组患者胃肠功能正常的时间、住院时间、手术过程出血量、术后平均引流量均明显优于开腹手术组(P<0.05)。结论：腹腔镜胃十二指肠溃疡穿孔修补的优势和手术效果确切，可有效减少出血，缩短术后胃肠功能恢复时间和出院时间，减少引流量和并发症发生，值得推广。

【Abstract】 Objective：To study the advantages and surgical results of laparoscopic perforation of gastroduodenal ulcer.Method：Forty-six patients with gastroduodenal ulcer perforation were divided into two groups from April 2015 to November 2016.The patients in the laparotomy group used the traditional surgery.The patients in the laparoscopic surgery group underwent laparoscopic surgery for laparoscopic gastroduodenal ulcer perforation.The two groups of patients with normal gastrointestinal function time，from the end of surgery to leave the hospital time，the amount of bleeding during surgery，postoperative average drainage and intestinal obstruction and other surgical complications were compared.Result：The complication rate of laparoscopic surgery group was significantly lower than that of laparotomy group(P<0.05).The normal time of gastrointestinal function，the time of hospitalization，the amount of bleeding during operation and the average postoperative drainage in the laparoscopic surgery group were significantly better than those in the laparotomy group(P<0.05).Conclusion：The advantages of laparoscopic perforation of gastroduodenal ulcer and the exact effect of operation can effectively reduce the bleeding，shorten the postoperative gastrointestinal function recovery time and discharge time，reduce the drainage and complications，it is worth promoting.

胃十二指肠溃疡为临床常见消化内科病，其发生和不规律饮食，如暴饮暴食、生活作息不良、精神紧张等因素相关，在长期胃十二指肠溃疡情况下，随着胃酸和胃蛋白酶侵袭作用影响，可出现胃十二指肠溃疡穿孔。胃十二指腸溃疡穿孔患者病情危重，甚至可出现休克、腹膜炎，严重威胁生命安全，在具备手术指征的情况下需及时采取手术治疗。目前临床常用的治疗胃十二指肠溃疡穿孔的手术方式有腹腔镜胃十二指肠溃疡穿孔修补术与传统开腹手术。其中，传统开腹手术治疗胃十二指肠溃疡穿孔修补可带来较大创伤，术后切口感染、肠梗阻等并发症风险较高，患者接受度较低[1]。腹腔镜胃十二指肠溃疡穿孔修补则凭借微创、安全、出血少、恢复快等优势受到临床广泛青睐和认可。本研究探讨了腹腔镜胃十二指肠溃疡穿孔修补的优势和手术效果，报道如下。, http://www.100md.com(郭朝晖)
