老年高血压性心脏病的超声诊断价值

【摘要】 目的 探讨超声心动图在老年高血压性心脏病的临床应用价值。方法 选取60~80岁高血压组患者37例，同年龄段非高血压组24例作对照，应用彩色多普勒超声对心脏进行探测，检查结果与非高血压组对照分析。结果 老年高血压组与非高血压组比较，心脏室间隔(IVS)、左室后壁(LVPW)明显增厚(P<0.01)，左房(LA)增大、主动脉(AO)扩张(P<0.01)，E/A明显减低，差异有统计学意义(P<0.05)。结论 超声心动图可作为老年高血压性心脏病的常规检查方法，对临床更准确地了解老年高血压患者心脏结构和功能的变化具有重要的诊断价值。

【Abstract】 Objective To explore the clinic value of the application of Ultrasound cardiogram in aged patients hypertensive heart disease(HHD). Methods 37 cases were selected from the hypertension disease patients who were age from 60 to 80, and 24 age-matched normal person were chosen in the study, Used the Ultrasound cardiogram to monitor the heart, compared and analyzed the difference between the groups of hypertension and nonhypertension.Results Compare the groups of aged hypertensive heart disease(HHD)and nonhypertensions, the heart IVS and LVPW of the aged patients withhypertensive heart disease(HHD)were thicker(P<0.01), and increased significantly in LA,distention of aorta(AO)(P<0.01), and E/A reduced significantly, there was statistical significance(P<0.05).Conclusion The Ultrasound cardiogram can be used as a regular checking method in aged patients hypertensive heart disease(HHD), and there is important clinic value in learning the changing of the heart structures and functions of aged patients hypertensive patients.

根据世界卫生组织(WHO)高血压的诊断，年龄60岁以上，在未使用抗高血压药物的情况下，血压值持续或非同日3次以上超过血压诊断标准，即收缩压≥140 mm Hg(18.6 KPa)和(或)舒张压≥90 mm Hg(12 KPa)者称为老年性高血压。随病程进展，血压持久升高，有心、脑、肾等靶器官受损的表现[1]，同时年龄增大亦可引起心脏结构及功能受损。本文旨在探讨老年性高血压心脏结构及功能变化，并研究与非高血压老年人的差异。成都市第二人民医院心血管内科胡咏梅
1 资料与方法
1.1 一般资料
高血压组：选取60~80岁原发性高血压组患者37例，平均年龄69.9岁，男22例，女15例，对象来源于住院及社区健康体检中发现的高血压患者，病程5年以上，病史均排除糖尿病、肥厚性心肌病、高脂血症等。
非高血压组：对象来源于社区健康体检中非高血压老年人24例，男性14例，女性10例，平均年龄69.2岁，年龄及性别与高血压组相匹配，经多项体格及实验室检查证实无糖尿病、肥厚性心肌病、高脂血症等。
1.2 超声检查仪器 东芝SSA-660A彩色多普勒超声诊断仪及东芝SSA-580A彩色多普勒超声诊断仪，相控阵探头，探头频率为2~3.5 MHz。
1.3 检查方法 超声心动图检查：患者取左侧卧位或平卧位，检查时嘱患者平静呼吸。①取胸骨旁左室长轴切面，于舒张末期二尖瓣腱索水平测量室间隔厚度 ......
