乳腺癌术后行自体脂肪移植的研究进展及安全评估(3)
4 前景
目前在全球范圍内，自体脂肪移植被广泛应用于乳腺癌患者的乳房重建。对乳房AFG肿瘤学安全性，临床数据和实验研究提供了相互矛盾的证据。临床应用自体脂肪移植随访后发现，除了使上皮内瘤变患者的肿瘤复发率升高，术后乳腺癌复发率与对照组相比，并没有明显差异。大量的基础研究表明，当脂肪干细胞与肿瘤细胞共同培养时，脂肪干细胞通过增强肿瘤细胞的侵袭力，诱导血管生成，可能会对乳腺癌的生长和转移发挥促进作用。笔者认为，之所以会产生相互矛盾的两种证据，一方面是由于自体脂肪移植和CAL中提纯的ASCs浓度远远低于基础研究所用的ASCs浓度；另一方面，由于随访时间较短，缺乏高质量的临床数据，还需5年以上的随访提供更为可靠的证据，并且许多临床研究都有自己的局限性，如对照组与实验组之间未充分匹配临床、病理特征，结果缺乏可比性。另外，乳腺癌复发多在术后早期发生，临床医生会选择与手术间隔一段时间后再进行乳房重建，这就使实验组的选择出现选择性偏倚。
目前，对自体脂肪移植的适用人群尚没有公认的标准。笔者认为，应严格筛选患者，术后严密监测潜在的复发，对有高危因素的候选人延迟脂肪移植、避免纯化的脂肪干细胞注射。虽然这些建议可以有助于提高自体脂肪移植的安全性，但对自体脂肪移植还需要更长的随访时间和更细致的实验设计，获得长期肿瘤学安全性的临床结果。此外，ASCs浓度对乳腺癌复发的影响还需要更多的基础研究来证明。
