负压引流技术对糖尿病足患者足部创面外观修复效果研究(1)

     [摘要]目的:分析负压引流技术(Vacuum sealing drainage, VSD)治疗糖尿病足的临床疗效及对足部创面外观修复效果,为糖尿病足治疗方案选择提供参考。方法:2016年1月-2017年1月笔者医院收治符合纳入条件的糖尿病足患者108例,按照治疗方案不同将其分为VSD组(52例)和常规治疗组(56例)。两组患者均予以糖尿病常规治疗,健康指导、控制血糖,清创换药治疗,VSD组在常规治疗基础上行负压引流治疗,连续引流24h,7d后拆除敷料。观察两组患者创面愈合时间、创面愈合率、有无暴露骨骼及肌腱、患足外观修复優良率,评价两组患者临床疗效及并发症发生率。结果:VSD组临床总有效率为84.61%明显高于常规治疗组的67.86%,差异有统计学意义(P<0.05)。VSD组创面愈合时间、创面愈合率、患足外观修复优良率及暴露骨骼、肌腱率均优于常规治疗组,差异有统计学意义(P<0.05)。两组患者并发症发生率比较,差异无统计学意义(P>0.05)。结论:应用负压引流技术治疗可有效提升糖尿病足患者足部创面外观修复效果,提高临床治疗总有效率,患者符合VSD手术指佂可优先考虑此种治疗方案,提升治疗综合质量。

    Abstract: Objective To analyze the clinical efficacy of vacuum sealing drainage(VSD) in the treatment of diabetic foot and its effect on the appearance of foot wounds, so as to provide a reference for the treatment of diabetic foot patients. Methods 108 cases of diabetic foot patients from January 2017 to January 2016 in our hospital were selected. And they were divided into the VSD group (52 cases) and the routine treatment group (56 cases) accorded to the different treatment plan. Two groups of patients were given conventional treatment of diabetes, health guidance, blood glucose control, debridement and dressing treatment. The VSD group was treated with negative pressure drainage on the basis of conventional treatment, and 24h was removed after continuous drainage, and the dressing was removed after 7d. The healing time of wound healing, wound healing rate, unexposed bone and tendon, and the good rate of foot appearance repair were observed in the two groups, and the clinical efficacy and complication rate were evaluated between the two groups. Results The total effective rate of the VSD group was 84.61%, which was significantly higher than that of the routine treatment group(67.86%), the difference was statistically significant(P<0.05). The wound healing time, wound healing rate, appearance and repair rate of the affected foot and the rate of exposed bone and tendon in the VSD group were all better than those of the routine treatment group(P<0.05). There was no significant difference in the incidence of complications between the two groups(P>0.05). Conclusion Vacuum sealing drainage can effectively improve the appearance of patients with diabetic foot wound healing effect, improve the total efficiency of clinical treatment, patients with VSD surgical indications for this therapy can enhance treatment in patients with limited consideration, comprehensive quality., 百拇医药(罗鹏)
