水动力清除系统在清除双侧注射式隆乳材料PAHG中的应用效果(1)

     [摘要]目的:探讨水动力清除系统在清除双侧注射式隆乳材料聚丙烯酰胺水凝胶(Polyacrylamide hydrophilic gel, PAHG)手术中的应用效果。方法:按照随机数字法将2016年1月-2017年12月在笔者医院进行PAHG取出术的65例患者分为研究组(n=34)及对照组(n=31)。研究组实施水动力清除系统结合腔镜PAHG取出术,对照组实施常规肿胀抽吸术。比较两组患者的住院时间、手术时间、引流时间、引流量、术后并发症发生率、假体残留率及患者满意度。结果:研究组的住院时间、手术时间、引流时间、引流量及并发症发生率与对照组比较,差异无统计学意义(P>0.05)。研究组术后假体残留率为14.71%显著低于对照组的61.29%,且研究组患者满意度为94.12%明显高于对照组的64.52%,差异有统计学意义(P<0.05)。结论:水动力清除系统清除双侧注射式隆乳材料PAHG能够降低假体残留率,提高患者满意度。

    Abstracts: Objective To discuss the applicative effects of hydrodynamic scavenging system in removal operation of bilateral injection augmentation mammoplasty material polyacrylamide hydrophilic gel(PAHG). Methods 65 cases of patients which was carried out PAHG removal surgery in our hospital from January 2016 to December 2017, were divided into the research group(n=34) and the control group(n=31). The patients in the research group were carried out PAHG removal surgery under the hydrodynamic scavenging system combined with endoscope, the control group were carried out conventional tumescence suction. The hospitalization time, operative time, drainage time, volume of drainage, incidence of complications, residual rate of prosthesis and patients' satisfaction rate was compared between the two groups. Results There was no significant difference in the hospitalization time, operative time, drainage time, volume of drainage and incidence of complications between the study group and the control group(P>0.05). The residual rate of prosthesis in the study group was 14.71%, significantly lower than that in the control group (61.29%), and the satisfaction rate of the study group was 94.12%, which was significantly higher than that of the control group (64.52%), the differences were statistically significant(P<0.05). Conclusion Hydrodynamic scavenging system could reduce the prosthesis residual rate and improve patients' satisfaction degree in removal operation of bilateral injection augmentation mammoplasty material PAHG.

    近年來,随着人们审美观的改变及提高,越来越多的女性选择美容整形,其中隆乳术得到广泛应用。聚丙烯酰胺水凝胶(Polyacrylamide hydrophilic gel,PAHG)是一种注射式软组织填充材料曾被广泛应用于隆乳术中[1]。但是随着研究的深入,有研究证实使用PAHG的隆乳术后并发症较多,如:乳房疼痛及硬结、乳房感染、填充材料移位等,由于对PAHG安全性考虑及术后并发症引起的心理压力[2-3],较多患者要求将PAHG隆乳材料取出。目前,临床上PAHG隆乳材料的取出方法较多,但均存在不同程度的不足及缺陷[4]。水动力清除系统是以水作为介质,通过给予额外的能量,让其达到刀样切割分离的作用,具有对周围组织创伤小、可有效改善术后生活质量等优点[5-6]。本研究将探讨水动力清除系统在清除双侧注射式隆乳材料PAHG手术中的应用效果,观察其是否可以较大程度地清除浸润病变组织、有效降低假体残留,保护周围组织,为临床提供参考。, http://www.100md.com(张姣姣)
