应用OPA-K直丝弓矫治器双颌减数治疗安氏II1类错牙合的临床研究(1)

[摘要]目的：评价OPA-K直丝弓矫治器双颌减数治疗安氏II1类错牙合的临床疗效，并探讨其机制和使用方法。方法：临床选择15～23岁安氏II1类错牙合患者22例，应用OPA-K直丝弓矫治器进行双颌减数治疗，并对治疗前后X线头影测量值的变化进行分析。结果：矢状方向上，U1-NA和U1-SN分别减小14.26°和10.32°，U1-NA(mm)减小4.16mm，U1-L1增加17.06°，以上差异均具有统计学意义(P<0.05)，而SNA、SNB、ANB、L1-NB、L1-MP的变化均无显著性(P>0.05)。垂直方向上，SN-MP和Y轴的变化均无显著性(P>0.05)。结论：应用OPA-K直丝弓矫治器治疗安氏II1类错牙合可取得令人满意的临床效果。

Clinical study of the Oriental Preadjusted Appliance-KOSAKA applied in Angle class II division 1 malocclusion with bimaxiliary extraction treatment

Abstract:ObjectiveTo evaluate the effects of the Oriental Preadjusted Appliance-KOSAKA applied in Angle class II division 1 malocclusion with bimaxiliary extraction treatment and discuss the mechanism and application.Methods 22 cases aged 15～23 years old with Angle class II division 1 malocclusion were chosen and treated with the Oriental Preadjusted Appliance-KOSAKA.Cephalometric analysis was performed before and after the treatment.ResultsIn the sagittal direction, U1-NA and U1-SN were reduced 14.26° and 10.32°,U1-NA(mm) were reduced 4.16mm,U1-L1 were increased 17.06°,there were significant differences in these changes(P<0.05).but the changes of SNA、SNB、ANB、L1-NB and L1-MP were no significant(P>0.05).In the vertical direction,the changes of SN-MP and Y axis were no significant(P>0.05).ConclusionThe Oriental Preadjusted Appliance-KOSAKA is effective for Angle class II division 1 malocclusion.

安氏II1类错牙合是临床常见的错牙合畸形，严重影响患者的容貌外观和口腔功能。其正畸治疗的目的是最大限度内收上颌前牙，获得良好的前牙覆盖、覆牙合关系，从而减少面中1/3突度，改善面部侧貌。当前，直丝弓矫治技术以其方便、高效和精确等优点已经得到日益广泛的开展和应用，并成为正畸临床主流的治疗技术。笔者应用由日本正畸医师小坂肇结合东方人(日本人)牙牙合面结构特征及矫治力学特点研究开发的东方人预成直丝弓矫治器(Oriental Preadjusted Appliance-KOSAKA,OPA-K) ......
